Study Description: This is a clinical research study to test the safety and effectiveness of an investigational medication as an add-on treatment for Chronic Obstructive Pulmonary Disease (COPD). You may qualify to participate if you have been diagnosed with COPD, are currently prescribed triple maintenance therapy and have had at least one exacerbation in the past year. You may be asked to attend up to two screening visits to determine if you are eligible to enroll in the study. If enrolled, you will be randomly selected to receive either the investigational medication, an approved oral medication (known as roflumilast, marketed as Daliresp), or a placebo to be taken daily for one year. Study-provided medication will be taken alongside your current triple COPD medications and rescue therapy will be available as needed. Your total participation will last just over 1 year (56 weeks), and you will be asked to complete 2 phone calls and attend up to 8 doctor's visits throughout the study. You will complete various diagnostic and laboratory tests during your visits, and you will be asked to complete daily questionnaires in an e-diary to evaluate your COPD symptoms. The doctor’s office conducting the study will be able to provide you with more information about the study requirements. Qualifications: - Must be diagnosed with COPD and chronic bronchitis for one year or longer - Must be treating your COPD using regular maintenance triple therapy (an inhaled corticosteroid (ICS), an inhaled LABA (a type of bronchodilator), and an inhaled LAMA (a type of bronchodilator) - Must have experienced a COPD exacerbation within the past year that required you to visit an ER and/or receive treatment with steroids or antibiotics - For current or former smokers - Cannot have a history of the following lung diseases: pulmonary fibrosis, cystic fibrosis, alpha 1 anti-trypsin deficiency, interstitial lung disease
